You must be a registered member of St. Maria Goretti Parish for your child to attend the school.

Applications are accepted November 1 through February 1.

Your child’s application is not complete until all supporting documentation is received:

• Copy of Birth Certificate
• Copy of Baptismal Certificate (and 1st Communion Certificate if applying to grades 3–8). If your child received sacraments at SMG, you do not need to upload the certificate(s).
• Copy of current immunizations. If your child is accepted and is due for any immunizations between now and the start of school, we will require a copy of the new immunization prior to the first day of school in August.
• Copy of most recent report card and standardized test scores (grades 1–8)
• A completed teacher questionnaire, returned to us by February 1

Teacher Questionnaire

Please give the appropriate form below to your child’s current teacher. The teacher completes it and returns it directly to the school — it does not come back through you.

• Teacher Questionnaire — Grades 1–8 (Fillable PDF) For students entering grades 1–8. Completed by the current K–7 teacher.
• Teacher Questionnaire — Kindergarten (Fillable PDF) For students entering Kindergarten. Completed by the current preschool teacher.

Please let your child’s teacher know to download the form before completing it — the fields will not accept typing in a browser or email preview window.

Teachers may email the completed form to [email protected], or mail it to St. Maria Goretti School, 17104 Spring Mill Road, Westfield, IN 46074.

After you apply

The application will be evaluated, and you will be notified by March 1 of your child’s status — either a seat in the class or a spot on our waitlist.

Children must be age 5 before August 1 to be eligible for our Kindergarten program.

For students entering grades 1–8, an appointment may be scheduled with the Admissions Team before enrollment.
